چکیده

Urgent upper gastrointestinal endoscopy was performed and documented a small esophageal varix without evidence of recent bleeding and, in the inferior wall of the second portion of the duodenum, a 20-mm elevated longitudinal lesion with a central hematinic depression, at that time misinterpreted as a subepithelial lesion with a visible vessel, and therefore, an endoclip was placed ( Fig. 1 ). Further investigation with endoscopic ultrasound (EUS) showed that the previously identified duodenal subepithelial lesion was in fact a 13-mm-large duodenal ectopic varix ( Fig. 2 a). Endoscopic injection of 4.8 mL of cyanoacrylate and lipiodol (0.5: 0.7) was successfully per

Idiopathic duodenal varix presenting as a massive upper gastrointestinal bleeding: A case report.

Ectopic variceal bleeding is both a diagnostic dilemma and a therapeutic challenge, especially when it is located in the third part of the duodenum. Varix is rare in the absence of cirrhosis or portal hypertension. Because the diagnosis of this condition is usually delayed, treatment is administered late resulting in high morbidity and mortality rate. We report a case of a 61-year-old lady with...

Cyanoacrylate glue was injected into the duodenal varix and successful obliteration of the duodenal varix was demonstrated on a foll...

Ectopic varices: Hidden challenges in the vascular anatomy.

A 50 yo female with PSC and a bleeding duodenal varix (DV) was evaluated for possible enbucrilate therapy. Endoscopically (Figure 1), the DV was high risk for rebleeding but the relative in-flow and out-flow were uncertain. CT (Figure 2) showed portal vein thrombosis (PVT) and the vascular anatomy of the DV to consist of inflow from the superior mesenteric vein (SMV) with outflow through the DV...
